Your Rights
As a client, you can expect Dr. Wellborn to provide compassionate, confidential, appropriate, and competent help with your problems or concerns. You can also expect to be informed of the benefits and risks of treatment.
Compassion. You can expect courteous, compassionate, and respectful treatment.
Confidentiality. The psychotherapy relationship is confidential. Information revealed within this relationship is between client and therapist. Dr. Wellborn makes every effort to maintain your privacy and confidentiality. You (or the parent or legal guardian of a minor) must give your permission before any information may be revealed to anyone else. However, there are some limitations to your right to confidential treatment:
- Your personal safety and that of other individuals is a high priority. If Dr. Wellborn has concerns about someone’s safety (suicide, homicide, abuse/neglect of children or dependent adult), reasonable efforts will be made to help keep everyone safe, even if confidentiality must be broken.
- Dr. Wellborn regularly consults with other therapists to insure the best treatment possible. Consultations are bound by the same confidentiality as that between you and Dr. Wellborn.
- Dr. Wellborn is required by law to disclose a client’s health information to authorized federal officials conducting national security and intelligence activities or providing protective services to the President or other important officials. It is unlawful for Dr. Wellborn to reveal that this information has been provided.
Appropriate Treatment. Dr. Wellborn will work with you to determine the most appropriate treatment for the difficulties you are experiencing. This includes developing a treatment plan, providing psychotherapy, consultations, or referring you to another professional with the appropriate expertise.
Competent Care. You have the right to competent, quality treatment that is consistent with professional standards established in practice and supported by research.
Informed Consent. You have the right to know the potential benefits and risks of psychotherapy in order to make an informed choice regarding your treatment. Dr. Wellborn is committed to using his skill and knowledge to help resolve any difficulties that arise. However, psychotherapy is not an exact science. While therapy most often improves or alleviates problems, there is still the possibility that you may experience negative effects from therapy despite your best efforts and those of Dr. Wellborn. Negative effects could include the worsening of problems, strained or damaged relationships with others during treatment, and the uncovering of unexpected, disturbing issues.
Additional Services
Dr. Wellborn provides fee-based services in addition to psychotherapy appointments that include telephone consultations, court appearances, treatment reports and summaries and school consultations. You will be informed before you are charged for these services.
- Phone consultations will be billed at Dr. Wellborn’s regular session fee. (Please note: Insurance may not cover these expenses.)
- Court appearance fees will be billed in 30 minute increments for time spent in preparation, travel, depositions, testifying, and writing reports. Payment will be expected in advance for all court-related services. (Please note: Insurance may not cover these expenses.)
- Treatment reports are often requested for various purposes such as disability claims or classroom modifications. Clients will be billed for the time spent preparing and writing the report. (Please note: Insurance may not cover these expenses.)
- School consultations will be billed in 30 minute segments at the full session rate including travel and consultation time. (Please note: Insurance may not cover these expenses.)
Cancellation Policy
Patients are seen by appointment only. Cancellations must be made at least one business day in advance of the scheduled time. After three same-day cancellations or not showing up for an appointment, clients will be discharged and therapy considered ended.

Dr. Wellborn is committed to providing excellent service. If you have questions or concerns about the service he provides, he will be happy to address them with you. For more serious concerns, you can also contact the Colorado Department of Regulatory Agencies Divisions of Professions and Occupations to file a complaint using their online complaint form. For Tennessee residents, you should contact the Tennessee Board of Examiners in Psychology at (800) 852-2187 for additional help or information.
